3. the question(s) justifying the scan,
4. timing expectations (days/weeks),
5. your name and address.

On the basis of this information, we will let you know :

1. whether the CT scan is likely to be useful, 
2. where and when we could scan the piece, 
3. the total price for either a  screening  or a  diagnostic  CT scan.
